No personal or identifying information is requested on this form. Once you complete the survey, it is uploaded to a spreadsheet that allows HLVLE to best assess the needs of our community. All survey participants remain anonymous.

Information about our members is legally protected. If you, as a member would like your information shared with another person or agency, a Release of Information (ROI) needs to be on file.
If you need help completing a ROI, please see a staff member.
